چکیده

INTRODUCTION The aim of this study was to investigate a single-electrode radio frequency ablation (RFA) needle as an instrument for liver resections with special emphasis on operation time, time of liver ischaemia, intra-operative blood loss and post-operative complications. MATERIAL AND METHODS A total of 40 consecutive patients having a liver transection performed by an RFA single electrode from 1 September 2011 to 28 February 2012 were included in the study. Data concerning type of liver resection, liver parenchyma transection time, intraoperative bleeding and transfusions were prospectively recorded and registered. Furthermore, complications were recorded with special emphasis on bile fistulas and abscesses. RESULTS In all, 20 females and 20 males had a liver resection performed by a single RFA electrode. The mean bleeding was 520 ml ± 469 ml, and the mean liver parenchyma transection time was 52 min. ± 22 min. Three patients, all of whom underwent major resections, received blood transfusions. Five patients developed bile fistulas and two abscesses. There were no re-operations for bleeding and no 30-day mortality. CONCLUSION A single electrode RFA needle is a suitable tool for liver parenchyma transection with regard to operation time and intraoperative bleeding, but the frequency of bile leakage seems to be unacceptably high in cases of hemi-hepatectomies. Robotic Assisted Radio-Frequency Ablation of Liver Tumors - Randomized Patient Study

The minimally invasive treatment of liver tumors represents an alternative to the open surgery approach. Radio-frequency ablation destroys a tumor by delivering radio-frequency energy through a needle probe. Traditionally, the probe is placed manually using imaging feedback. New approaches use robotic devices to accurately place the instrument at the target. The authors developed an image-guide...

An Augmented Reality Application for the Radio Frequency Ablation of the Liver Tumors

Hepatic cancer is one of the most common solid cancers in the world. As surgery of hepatic cancer is seldom applicable, different solutions have been found to cure this disease. One of these is Liver Radiofrequency Ablation. This technique consists in a needle insertion inside the liver parenchyma in order to reach the tumor and in an injection of a radiofrequency current to cause tumor cell ne...

Nodules in the Cirrhotic Liver

Cirrhosis is a common liver disease that is becoming even more prevalent due to the rapid worldwide increase in the incidence of hepatitis C. One of the most important complications of cirrhosis is the development of hepatocellular carcinoma (HCC). Unlike many other malignancies, HCC has a low rate of metastasis when detected early, when it is well circumscribed and 5 cm or less. Additionally, ...

Use of a New Algorithm with an Internally Cooled Electrode for Radiofrequency Ablation of Small Hepatocellular Carcinomas

Purpose: To investigate the efficacy of a new algorithm to increase the volume of tissue ablation via gradual ramp-up of power using an internally cooled electrode for ablating hepatomas 3 cm or less. Materials and Methods: 44 patients with 62 hepatomas were treated from March 4, 2004 to May 24, 2009. Ablation with a gradual ramp-up of power was performed using a single needle with an internall...

RF Ablation at Audio Frequencies Preferentially Targets Tumor - A Finite Element Study

Radio Frequency (RF) ablation has become of considerable interest as a minimally invasive treatment for primary and metastatic liver tumors. Major limitations are small lesion size, which make multiple applications necessary and incomplete killing of tumor cells, resulting in high recurrence rates. RF ablation is typically carried out in the frequency range of around 500 kHz. Measurements have ...
